Practice Transformation

The PT-SC is designed to collect descriptive PT site-level data for all PT sites during the reporting period (July 1 – June 30). An entry should be completed for each participating PT site. Please complete this form at the end of the reporting period.

Reporting timeline:
March – June: partners review PT-SC form and begin entering PT data
June 1 – 15: partners complete entry of all PT data
June 16 – 30: data and evaluation team performs final review and cleaning of data
After June 30: data is finalized and may not be changed. It will be submitted to HRSA with other required information.

The PT-SC form is separate from the reporting of PT events. PT events should be entered into the event record, with the funding marked as “Base PT” (or “MAI PT” for OUHSC’s Cherokee Nation PT site only). All events with PT funding must also have the PT clinic indicated.
