Step 1: Add a New Survey. Each clinic should have a PT-OA and PT-PM Baseline survey before PT activity begins. They should then complete both surveys annually following the baseline survey.
Step 2: Fill out as much of the survey as possible. At minimum, select the Clinic & Phase then click "Submit". This will begin the survey process.
Step 3: Send to Clinic/Complete Survey Click "View & Edit Survey Details" below the Clinic Information Card to access the Survey Link created in Step 2. That link can be sent to the Clinic for completion or completed (promptly) at a later date.
